This website does not request health information
Madox Medical is currently under construction and not yet open to patients. This site is informational only. It does not ask for personal health information such as symptoms, diagnoses, medications, or your Alberta Health Card number, and you should not include such details when contacting us. No medical or clinical services are provided through this website.
Information we collect
When you use the contact and opening-updates form on this site, we collect only the details you choose to provide:
- Your name
- Your email address
- Your phone number (optional)
- A subject line (optional)
- The message you write to us
We do not knowingly collect personal health information through this site. The message field is free text, so please do not enter health details there.
How we use your information
- To respond to your inquiry once our intake team is ready
- To add you to the opening-updates list, if you ask to be notified
- To let you know when the clinic, onsite ultrasound, and onsite pharmacy open
We do not sell your information, and we do not use it for advertising.
Storage, service providers, and retention
Form submissions are stored securely and trigger an email notification to our team. We use trusted third-party providers to operate the website and deliver these emails, and an embedded map from Google to show our location. These providers process limited information on our behalf for these purposes only.
We keep the information you submit so we can respond to you and manage the opening-updates list while the clinic prepares to open. You can ask us to remove your information using the contact details below, and we will action your request.
Website analytics
Madox Medical uses Google Analytics only after you choose "Accept analytics" in the cookie banner. It helps us understand aggregate website usage — such as which pages are visited — so we can improve the website. Information entered into contact, booking, or medical forms is not intentionally sent to Google Analytics.
You can decline analytics, or change your selection at any time using the "Cookie preferences" link in the website footer. If you withdraw consent, analytics collection stops.
Your choices and rights
Under Alberta's Personal Information Protection Act (PIPA), you may ask to access, correct, or delete the personal information you have provided, or ask to be removed from the opening-updates list. To make a request, email us at the address below.
Once the clinic opens and begins providing care, personal health information held by the clinic will be handled in accordance with Alberta's Health Information Act (HIA). A separate notice covering patient health information will be made available at that time.

This website and its contact form are not monitored 24/7 and must not be used for urgent medical needs. For a medical emergency, call 911 or visit your nearest emergency room.
